Fallon Health names Greg Woodsum as site director of its Summit ElderCare® PACE Center in Lowell

Worcester, Mass., March 12, 2024— Fallon Health is pleased to announce the recent hire of Greg Woodsum as site director of its Summit ElderCare program in Lowell. In his role, Woodsum will oversee the daily operations of the organization’s Program of All-Inclusive Care for the Elderly (PACE) Center located on the D’Youville campus. Among his responsibilities, Woodsum will lead a Care Team that includes a range of professionals who specialize in geriatric care and is responsible for ensuring that each participant’s personalized care plan allows them to live as independently as possible. He will also interact with family caregivers who play a critical role in the PACE model.

Woodsum has more than 20 years’ leadership experience in health care operations, with his most recent role being that of executive director at Rehab Without Walls, a multi-site residential rehabilitation program in New Hampshire for individuals with traumatic brain injuries. He is a licensed physical therapist and holds a master’s degree in management and health care administration.

PACE is a nationally recognized program and model of care that allows people to live independently in their homes and communities instead of a nursing home, while providing a better quality of life with dedicated caregivers, transportation, full medical care, and opportunities for socialization. PACE helps older adults avoid isolation and live full lives with all their care needs met.

Backed by nearly 30 years’ experience in PACE, Fallon Health has grown its Summit ElderCare program to today include five PACE Centers. Located in the communities of Lowell, Leominster, Springfield, Worcester, and Webster, the program serves approximately just under 1,400 individuals in 10 counties across the Commonwealth. Fallon Health’s PACE is the largest in New England and among the largest in the country. The not-for-profit health care services organization also operates a program in Western New York.
